Job Description

GENERAL SUMMARY:

The purpose of the Acute Care Service Coordinator [role] is to partner with the IAC Medical Director in building one world-class integrated acute care team across the care continuum which creates safe & controlled care transitions and implements high functioning and cost-positive protocols & quality measures. The role is composed of three fundamental capabilities: practice management, opportunity identification, and model implementation / change acceleration. 

ESSENTIAL JOB FUNCTIONS:                                                                                                                                            

 

1.       Supports Medical Director in deploying a well-structured site operating rhythm for the IAC team (Physicians, APPs, Scribes, Administrative Assistant) including but not limited to:

a.        Daily provider census driven workflow process

b.       Rounding / Discharge Huddles and Care Coordination

c.        Support across ED, HM and Post-Acute

d.       Driving site process around accountable task management (Action Register) and delivery dates

e.       Reinforcing clear expectations with clinical team

 

2.       Work with site administration and clinical team to identify, plan, and drive improvements in a project management environment such as;

a.        Readmission reduction

b.       ED time and efficiency metrics

c.        Patient experience

 

3.       Quick to identify issues and develop thoughtful opinions; data analysis as well as the ability to navigate the hospital organization expertly and bring resources to the table

 

4.       Act as an agent of change and become the hospitals point of contact for difficult -to-manage situations

 

                               

KNOWLEDGE, SKILLS AND ABILITIES:

 

Execution: 

 

·         Effectively identifies metrics at risk and confirms action plan in place.

·         Ensures data-driven approach to identifying/prioritizing aspects of a problem.

·         Executes in an organized and effective manner (with minimal instruction, oversight and re-work) and does so within the allotted time.

·         Maintains a high say/do ratio and drives accordingly via direct / indirect influencing … does not let things fall thru the cracks

·         Actively monitors action plans to ensure issues are addressed and changes are instituted.

 

Clear and Concise Communication:

 

·         Ensures timely and clear communication with clinical leadership, hospital administration and other site personnel

·         Provides clearly structured written communications.

·         Prepares easily understandable documents that facilitate timely and efficient project work & decision making

·         Uses the appropriate communication style to clearly articulate issues/ideas/solutions/progress at all levels.

·         Able to challenge the status-quo and suggests changes and improvements to existing processes and procedures. 

·         Quick to leverage critical thinking skills in summarizing problem statements and arriving at core business needs

·         Works as a dyad partner with Integrated Acute Care Site Medical and Quality Director

·         Identifies own weaknesses and seeks opportunities to develop needs … Goes outside comfort zone.

Cedar Park Regional Medical Center is a brand new, state of the art facility in a quickly growing area. This new facility was designed around the entire patient and visitor experience to offer a soothing place of healing along with the latest technological advances. Its services range from advanced surgical suites to incredible home-like labor and delivery suites for expectant parents. Hospital includes 151 beds, Cath Lab, digital CT, MRI, Ultrasound and Radiology, as well as wireless instant nurse-to-nurse communication. 

Job Requirements

EDUCATION AND EXPERIENCE:

 

  1. BSN required.
  2. Five or more years of relevant work experience with emphasis on performance improvement, data and project plan management.
  3. Clinical experience ie; Emergency / Outpatient, Inpatient and Post-Acute settings a plus.
